The Center is excited to provide Collaborative Documentation (CD) training for all direct service staff! The Center will now utilize CD across all programs and staff as the manner in which we document clinical interactions with clients/consumers – this is how documentation will now be done. CD is an evidence informed clinical practice in which the clinical staff documents all aspects of the clinical record with the client/consumer present. The client/consumer is privy to all documentation content and is afforded the opportunity to have their perspective documented as well. Collaborative Documentation has multiple demonstrated benefits including: improved client engagement; reduced no shows and cancellations; improved medication adherence; and support for sustainable compliance of core clinical documentation elements (assessments, treatment plans, progress notes). It also creates substantial clinical capacity, allows for real time documentation completion that supports care coordination and risk management, improves accuracy of records, and, it enhances the quality of clinical staff’s work life.
The CD Train the Trainer is designed to support organizations in their ability to provide their own internal CD Training for new staff and as a refresher for previously trained staff. This program is also helpful to supervisors who will need the capacity to coach their clinical staff in maintaining quality Collaborative Documentation ongoing.
